We work closely with clients, families, and care providers to deliver personalised assessments and practical solutions.

Comprehensive Home and Functional Assessments 

Comprehensive assessments and reports to identify functional status, environmental barriers, and safety concerns, to determine necessary support needs for improving independence and quality of life.

Equipment Prescription

Prescription of basic and complex assistive equipment such as aids for activities of daily living, bathroom equipment, chairs, beds, and personal alarms. Collaboration with equipment suppliers and home care providers to ensure recommendations are functional, cost-appropriate and compliant.

Wheelchairs and Seating

Comprehensive support for selecting and using appropriate mobility devices, including manual and powered wheelchairs. Services include mobility trials, training, and prescription to maximise safety, independence, and confidence in daily mobility.

Manual Handling and Falls Prevention

Home Modifications

Assessment, design recommendations, and reports to support minor and major home modifications — such as grab rails, ramps, bathroom alterations, and access improvements — tailored to each client's physical and cognitive needs. Collaboration with builders and home care providers to ensure recommendations are functional and compliant.

Pressure injury management and prevention

Specialised assessment of pressure risk and injuries, prescription of specific equipment and aids such as cushions, mattresses, and seating options to best manage pressure care needs.

Cognitive and Dementia Support

Targeted assessments and interventions to enhance memory, safety, routines, and independence for individuals with cognitive impairment or dementia and their carers.

Assessment and training on safe manual handling techniques to reduce injury risk for clients and carers. This includes transfer training, equipment recommendations, and home set-up advice. Our falls prevention services identify individual risk factors and provide tailored strategies to improve safety, mobility, and confidence at home.
